Hence, I ordered the Self Journal from Amazon in the color Charcoal. It is a daily planner for 90 days, and includes places for you to track your weekly progress towards your overarching goal. I just started using it, but it seems very well thought-out in terms of the structure and seems like it aligns well with the psychology behind what we need to achieve our goals.
